Health Links was a program developed by the Ministry of Health to address the needs and gaps in the health system for the 5% of patients who account for 2/3 of healthcare costs. It is a system of more coordinated care where the hospital, family doctor, long-term care home, and community organizations work together as an integrated team. Health Links provide a care plan for each patient and work together with patients and their families to ensure they receive the care they need.
This project was highly influential with decision makers in showing the impact of Health Links by focusing on a series of patient stories.
